PREP Registration Form

Fees

• 75.00 per 1st-8th grade child
• Additional $25.00 Sacramental Fee for children in Second or Eight grade
• Maximum charge will be for 2 children. If 3+ children in prep - no additional charges for 3rd child and beyond and there are no fees for children attending that are younger than Kindergarten

All NEW registrations 1st-8th grader must be accompanied by a certificate of Baptism

• Once completed, drop it off at the parish office or email to prep@htassumption.org.
